REQUIREMENTS FOR EQUIPMENT RENTAL

1. IDENTIFICATION

Valid Drivers License, Passport or other Government Issued Picture ID

2. INSURANCE / COLLATERAL DEPOSITS:

CASH or CREDIT CARD deposit for the full replacement value of equipment -OR- CERTIFICATE OF INSURANCE (See Insurance Requirements) and a CREDIT CARD deposit for deductable Note: CHECKS, CHECK CARDS and DEBT CARDS are NOT ACCEPTED for Collateral Deposit

3. CREDIT CARD AUTHORIZATION FORM

For agreed upon payments, deductibles, and to guarantee payment (We accept VISA, MASTERCARD and AMERICAN EXPRESS only) PHONE ORDERS require legible copies (Front and Back) of card holder’s DRIVERS LICENSE and CREDIT CARD

RENTAL AGREEMENT explains our terms and conditions for rental.

BUSINESS HOURS: 8:00AM TO 5:00PM Monday through Friday, except holidays. We are accessible after hours. After hour deliveries and other services will be charged at an overtime rate.

RENTAL PERIOD: The minimum rental period is one day. Equipment must be returned by 10 A.M. to avoid an extra day charge. Special weekly package rates available.

CANCELLATIONS: 24 hour notice is required of all cancellations. The renter shall compensate JR LIGHTING, INC. for any loss it may sustain as a result of the renter canceling all or part of an order.

INSURANCE: Renters shall provide their own insurance to cover the retail value of equipment rented for all loss and damage. Rental cost will not be deducted from replacement cost. Certificates of insurance, or verification from insurance company that they are forthcoming, must be made available to JR LIGHTING, INC. prior to release of equipment.

LIABILITY: JR LIGHTING, INC. Is in no way responsible for any liabilities, claims, costs, or expenses arising out of the use or possession of its equipment. Our equipment is offered for inspection and test at the time of rental.

DAMAGED EQUIPMENT: All equipment is rented in good condition and is to be returned in the same condition. Necessary repair work shall be paid for by the lessee and rental costs will continue until unit is returned in satisfactory condition. If unit is damaged beyond repair lessee shall be charged the full list price.

GLOBES: All units are rented with globes. All burnouts and broken globes will be charged for.

RENTALS: All rentals are subject to all the terms and conditions of the JR LIGHTING, INC. rental contract. Copy available upon request.

EXPENDABLES: All expendables are available for purchase. Truck packages include expendables, charged as used. Special order items will be purchased.
